QWEST PRIZE A DAY SWEEPSTAKES OFFICIAL RULES
NO PURCHASE NECESSARY TO ENTER OR WIN. A PURCHASE WILL NOT INCREASE YOUR CHANCE OF WINNING: The Qwest Prize A Day Sweepstakes begins at 12.01 a.m. Mountain Daylight Time (MDT) on June 1, 2008, and ends at 11:59 p.m. MDT on July 19, 2008 ("Promotion Period"). The Sweepstakes is sponsored by Qwest Corporation ("Sponsor"). There will be 7 Weekly Drawing Periods during the Promotion Period, each beginning on the applicable Sunday at 12:01 a.m. MDT and concluding at 11:59 p.m. MDT the succeeding Saturday.
ELIGIBILITY: This Sweepstakes is open only to legal residents of AZ, CO, IA, ID, MN, MT, ND, NE, NM, OR, SD, UT, WA and WY who are 18 years or older at the time of entry . Employees, officers and directors of Qwest, Group O Direct,, and their affiliates, advertising and promotional agencies and their immediate families (spouse, siblings, parents and children and their spouses) and members of their immediate households are not eligible. This Sweepstakes is void where prohibited.
TWO WAYS TO PARTICIPATE:
ENTRY METHOD 1:
During the Promotion Period, eligible individuals who subscribe online (through the qwest.com website) to Qwest High-Speed Internet® service will automatically be entered into the sweepstakes. The date the order is placed will be considered the sweepstakes entry date.
ENTRY METHOD 2:
All other eligible entrants (non-Qwest customers, Qwest customers who do not wish to subscribe to Qwest High-Speed Internet service, and Qwest customers who already are Qwest High-Speed Internet customers) may receive one sweepstakes entry by visiting www.qwest.com/prizeaday (the "website") during the Promotion Period and completing and submitting the official online entry form . Failure to provide complete information will result in automatic disqualification.
Entries must be received during the applicable Weekly Drawing Period to be eligible for a given Weekly Drawing . Sponsor and its agents are not responsible for entries that are lost, late or illegible, regardless of the reason, of for late or lost regular or express mail. All entries become the property of Sponsor and will not be returned. Limit one entry per person or email address during the Promotion Period, regardless of the method of entry. In the case of multiple entries, only the first entry will be accepted.

WINNER SELECTION AND NOTIFICATION/ODDS: Daily winners will be selected in a random drawings from all eligible entries received during Weekly Drawing Periods. Seven winners will be selected in each weekly drawing, one winner from among entries received on each day of the week, to award the specific prize for that day. For example, for all entries received on Monday, June 1, 2008, one winner will be selected on June 13, 2008 to be awarded a Garmin Nuvi 660 GPS, along with one winner for of the specific prize for each other day of that week.

Drawings will be conducted by Group O Direct, an independent judging agency. Winners will be notified by a traceable form of delivery and/or telephone and will be required to complete, and sign an Declaration of Eligibility, a Liability Release and, where lawful, a Publicity Release to be returned within ten (10) days of notification. Should a winner not be of the legal age of majority (a "minor") in state of residence, documents will have to be ratified and signed by parent or legal guardian. Failure to timely return documents or return of any notification as undeliverable will cause prize to be forfeited and an alternate winner will be selected. Odds of winning depend upon the number of eligible entries received for a drawing. Limit: One prize per household during entire Promotion Period.
PRIZE RESTRICTIONS: is not transferable or redeemable for cash; and prize substitutions will not be allowed, except at discretion of Sponsor. Any taxes on the prize and any expenses not specified relating to the acceptance and use of a prize shall be the sole responsibility of the winner.
GENERAL CONDITIONS: By entering, participants (and their parent or legal guardian if a minor in their state of residence) agree (a) to these Official Rules and the decisions of the Sponsor and the judging agency,, which shall be final in all matters relating to the Sweepstakes; (b) to release and hold harmless Sponsor, Group O and their respective affiliated companies and directors, officers and employees from any and all claims, liability or damages arising out of their participation and the acceptance and use of a prize; (c) if a winner, to permit Sponsor and its agents to use winners name, city and/or likeness for advertising, promotional and publicity purposes in all media without additional compensation and any prior review or approval; (d) that Sponsor has made no representation, warranty or guaranty relative to any prize; (e) that under no circumstances will participant be permitted to obtain awards for, and participant hereby waives all rights to claim punitive, incidental, consequential or any other damages, other than for actual out-of-pocket expenses; and ((f) that all causes of action arising out of or connected with this Sweepstakes, or any prizes awarded, shall be resolved individually, without resort to any form of class action. This Sweepstakes is governed by laws of the State of Colorado with venue in Denver, CO for the resolution of all claims and disputes.
Sponsor and judging agency are not responsible for printing or typographical errors in any Sweepstakes-related materials; for stolen, lost, late, misdirected, incomplete, or illegible entries, or for transactions, entries or communications that are processed late or incorrectly or lost due to computer or electronic malfunction or human error.
CAUTION: ANY ATTEMPT BY AN ENTRANT TO DELIBERATELY DAMAGE ANY WEB SITE OR UNDERMINE THE LEGITIMATE OPERATION OF THIS PROMOTION IS A VIOLATION OF CRIMINAL AND CIVIL LAW. If, for any reason, the Sweepstakes cannot be conducted as planned by reason of infection by computer virus, unauthorized intervention, technical failures, or any other causes which, in the sole opinion of the Sponsor, corrupt or affect the administration, security, fairness, integrity or proper conduct of this Sweepstakes, the Sponsor reserves the right in its sole discretion to cancel, terminate, modify or suspend this Sweepstakes and may conduct a drawing and select a winner from among all eligible entries received for that drawing prior to the action taken.
